Forms due before the Internship Begins

Student Internship Agreement

This form details the rules and regulations about insurance and personal conduct.

Internship Information - Form 100

This two-page form contains all primary information about the internship and the guidelines and policies of the Westminster Internship Program. Due before the beginning of the internship. *Note - the price per credit for Summer has changed.  See form for details.

International Students -  Fill out Form 110 () instead of Form 100.

Course Description - Form 399

This form is the permanent record of your credit-worthy experience. Due by the end of the first week of the internship or by the final drop date for the semester, whichever is earlier.

The Learning Contract - Form 200

The contract is written by the student in consultation with both the faculty sponsor and site supervisor stating the learning goals and strategies. Due by the end of the first week of the internship or by the final drop date for the semester, whichever is earlier.

Forms due during the Internship

Mid-Internship Self-Performance Evaluation - Form 301

The intern evaluates their own experience.  This form is due at the mid-point of the internship.

Intern Self-Performance Evaluation - Form 401

The intern evaluates their performance. This form is due at the end of the internship.

Student Internship Evaluation - Form 500

This form evaluates the Internship  Program as a whole and is handed into the Internship Coordinator at the exit interview

Employer Forms

Mid-Internship Peformance Evaluation - Form 300

The site supervisor evaluates the performance of the intern. This form is due at the mid-point of the internship.

Internship Performance Evaluation - Form 400

The site supervisor evaluates the performance of the intern. This form is due at the end of the internship.
