Students who are U.S. citizens or who have domestic classification and have health insurance coverage through another entity (parents, spouse, etc.) will not be required to buy the College-sponsored plan, but must opt out online before 8/31/13. 

To opt out, the student need only complete the online form providing proof of other health insurance coverage at www.summitamerica-ins.com after April 15, 2013 until August 31, 2013 deadline (January 31, 2014 deadline for Spring transfers) and at the beginning of each school year thereafter that the students does not need insurance.

• Student accounts will automatically be billed for the cost of insurance after the deadline of August 31, 2013, unless the student has opted out.   There will be no refunds given after the deadline date.

REQUIRED HEALTH FORMS  

This form must be completed and submitted to The Wellness Center BEFORE the first day of class attendance. Please be sure to submit the required immunization documents, which are listed in the health form.
